The North Carolina Council on Developmental Disabilities (NCCDD) is planning for its next Five-Year State Plan. The plan will begin October 1, 2027 and end September 30, 2031. Right now, NCCDD is asking for people to complete a survey to help NCCDD find out what should be included in the Plan.

What does the Five-Year State Plan do?
The plan lists the goals and initiatives that NCCDD will support to help improve services and supports for North Carolinians with intellectual or other developmental disabilities (I/DD).
Why should people take the survey?
The survey asks questions about things people need to be able to be a part of their community. The answers will help NCCDD to decide what kinds of projects it will fund to improve the lives of people with I/DD.
Who can take the survey?
People with I/DD, their families, supporters, and other stakeholders living in North Carolina.
How can I share the survey?
First, share the links for the survey. Surveys are available in English and Spanish. There are three ways to take the survey: online, or by downloading a PDF or Word document. Here are the links to the different surveys.
