Working Together to Turn Possibilities into Opportunity during the Transition to Adulthood

Work Together NC is a statewide initiative focused on the transition to adulthood for people with intellectual and developmental disabilities. The presenters will provide an interactive session on Work Together NC’s Action Plan Tool, covering key areas of the transition to adulthood such as employment, postsecondary education, navigating adult services, and guardianship decisions. The presenters will demonstrate how to use these plans to support an individual’s goals for adult life. Throughout the session, participants will have an opportunity to provide feedback to support ongoing improvement of the customizable Action Plan tool.
